NFL on FOX Insiders Mike Garafolo and Peter Schrager have both confirmed through sources that Chuck Pagano's extension with the Colts is for four years.

Chuck Pagano staying put in Indy (9:45 p.m. ET)

What was long thought a foregone conclusion, Chuck Pagano's ouster in Indianapolis, is not happening. Instead, the club announced late Monday night that it has reached a contract extension with its embattled head coach.

Chargers fire OC Frank Reich (3:48 p.m. ET)

Despite piloting a San Diego offense that was ravaged with injury, the Chargers have fired offensive coordinator Frank Reich, according to the San Diego Union-Tribune. Just last year, Reich was a hot name to be a head coach as he interviewed with the Jets and Bills.

Dolphins interview Mike Smith for head coach (3:13 p.m. ET)

The Miami Dolphins aren't wasting any time. Former Falcons head coach Mike Smith interviewed for the Dolphins' vacancy on Monday, according to the Miami Herald.

Report: Giants GM Jerry Reese 'safe' in New York (2:52 p.m. ET)

In what was largely an expected move, NFL Media's Aditi Kinkhabwala reports the Giants' longtime general manager will remain in place despit firing Tom Coughlin.

Tom Coughlin steps down as Giants head coach, doesn't intend to retire (2:27 p.m. ET)

The New York Giants are undergoing a coaching change.

Tom Coughlin has informed his coaching staff he is stepping down. FS1 insider Mike Garafolo noted that Coughlin's exit will be some form of resignation, but the 69-year-old coach doesn't intend to retire. If the right opportunity presents itself, Coughlin will coach again, according to Garafolo.

49ers will retain GM Trent Baalke (1:40 p.m. ET)

San Francisco 49ers head coach Jed York announced the team will keep general manager Trent Baalke as the team's general manager. "Trent and his staff will have all the resources at their disposal to get us back where we belong," York said. "Before we turn to 2016, we need to put 2015 behind us. I want this team to win. Nobody wants this team to win more than I do."

Chargers officially keeping Mike McCoy in 2016 (12:55 p.m. ET)

The team has informed Mike McCoy that he'll be retained for the 2016 season, the team announced on Monday. McCoy, who is entering the final year of his deal, finished 4-12 this season and went winless in the AFC West.

Dolphins eyeing Josh McDaniels, Matt Patricia, Anthony Lynn (12:48 p.m. ET)

The Miami Dolphins have furiously begun their head coaching search. Newly tabbed GM Chris Grier has requested interviews with Patriots offensive coordinator Josh McDaniels, Patriots defensive coordinator Matt Patricia and Bills RBs coach/assistant head coach Anthony Lynn, according to ESPN's Adam Schefter.

FOXSports.com has learned that Mike Shanahan will meet Tuesday with Dolphins officials as the team seeks to fill the vacancy created by the in-season firing of Joe Philbin.

What's next in Indy? (8:28 a.m. ET):

It seems we should know Chuck Pagano's fate before too long. The Indianapolis Colts coach is scheduled to meet with team owner Jim Irsay this morning.

Dolphins starting early (Monday 8:02 a.m. ET):

Alex Marvez reports that Dolphins interim coach Dan Campbell will get a chance on Friday to interview for the job on a permanent basis.

The team is expected to conduct an exhaustive search and the 39-year-old former tight ends coach is expected to be a long shot to be hired.

UPDATE (Monday, midnight): Black Monday is officially upon us, and the first name to enter the fray is … Adam Gase?
