The 17-year-old, who can play in defence or midfield, captained the Blues to FA Youth Cup success last term and has also skippered the Premier League club's reserve side on a regular basis.
Chalobah, an England Under-19 international, is yet to make a first-team appearance for Chelsea but has been an unused substitute.
Meanwhile, Watford centre-back Tommie Hoban, 18, has followed in the footsteps of midfielder Sean Murray by penning a new five-year deal at Vicarage Road.
Elsewhere, defender Dale Bennett has joined AFC Wimbledon on a month-long loan deal while Adam Thompson has linked up with fellow League Two outfit Wycombe for the same spell.
